We will describe the development, implementation and evaluation of a DIVA pathway incorporating a DIVA identification tool and escalation strategy. The overall aim of the study entitled ‘DART3’ (Difficult Access Requires Thought, Training and Technology) study is to implement effective DIVA identification and escalation pathways to support PIVC insertion in adults and children, including appropriate use of ultrasound-guided procedures. The presentation will comprise:

1. A brief outline of DIVA risk factors and current practice to identify and manage DIVA.
2. The proposed solution, DART3 study design, DIVA tool development and implementation activities
3. Study findings and recommendations for sustainable implementation and the potential clinical and research applications.

To date DART3 has collected over 1,300 observations of PIVC insertions with over 60% of these identified as DIVAs or potential DIVAs. Early data suggests that the escalation pathway increases first attempt success for all patient groups, over 40% in DIVA patients. First attempt success also increases in all patients with advanced inserters, over 30% in DIVA patients.

  • Describe the risk factors and current tools to identify patients with difficult intravenous access
  • describe current barriers and enablers to widespread ultrasound adoption
  • Describe clinician and consumer experiences with DIVA
  • By the end of this session participants will understand vital components of the clinical escalation pathway for patients with DIVA including what makes and advanced inserter and use of ultrasound
  • Describe implementation strategies which aided the implementation of a hospital clinical assessment and escalation tool for patients with DIVA.
