Patients experiencing a 6-week course of IV antibiotic therapy in a unified healthcare system, cross paths with many nurses in many practice environments from hospital to Ambulatory Outpatient to Home Infusion. Patients also have a hand in the care of their line as well. We will discuss recommendations to promote positive clinical and vascular access patient experience outcomes through effective care coordination, communication and practice decisions.

Understand how a health care organization can apply their policies and procedures, didactic education and competency maintenance to provide consistent care across the continuum of health care settings.
Summarize potential locations in the healthcare experience where nursing and interdisciplinary processes impact the vascular access patient outcome.
