(M4) CLABSI Reduction in the ICU: Is It Time to Update Your Bundle?

This session will review strategies to mitigate central-line associated bloodstream infections (CLABSIs) within a high-risk setting in the intensive care unit (ICU). We will discuss the clinical setting, team structure, need for multidisciplinary clinical representation (e.g., nursing, physicians, vascular access specialists, data analysts, infection preventionists, and executive leadership), and implementation of a creative bundle to lower infection rates. In 2021, our 40-bed ICU had a CLABSI rate of 1.17, with a total of eight CLABSIs. Seven of our CLABSIs were attributed to common commensals (i.e., Staphylococcus epidermidis and Staphylococcus hominis), and one to Enterococcus faecalis. Additionally, 88% of our CLABSIs were attributed to patients with coronavirus disease 2019 (COVID-19). This being the highest CLABSI rate ever seen for our ICU prompted the creation of a vascular access committee, and the creation of the CLABSI Preventative Initiative for COVID Positive ICU (CPI2) bundle. The bundle was implemented in February of 2022, and the ICU achieved s total of 318 consecutive days without CLABSIs.
