A patient presented with ongoing challenges for vascular access and multiple failed attempts and use of PICCs, midlines, tunneled and implanted CVADs. Vascular surgery developed an arterio-vascular fistula (AVF) in the patient’s leg for access; a unique situation that required a lot of education and planning for patient care. This presentation will provide the process and work done to provide proper and safe care to this patient. A care plan and new policy and procedure was developed with minimal guidance, standards or precedence for this type of access to be used just for infusion therapy and outside of hemodialysis. Details on accessing and using the AVF for infusion therapy and medication administration will be shown using graphics, video clips as well as providing the experiences and perspectives of the patient and nurses involved. The case report here will present how this is a success story and may be considered in future for patients with challenges with vascular access.
